Thanks for your interest in the Family Satisfaction with Intensive Care Unit Questionnaires. With Dr. Heyland’s impending retirement, we have retired this website and transferred the housing, licensing, and access to all the FS-ICU questionnaires (all versions and translations) to Mapi Research Trust.

Therefore, all requests for information have to be submitted through the ePROVIDE™ platform.

  2. If you haven’t registered yet, you’ll be asked to sign up for free
  3. Complete the request form. You may attach documents if need be

Mapi Research Trust PROVIDE team will handle your request and get back to you.

Please specify in your request if the study is funded or not. Note that users who do not receive specific funding for their use of the questionnaire can download the COAs distributed by Mapi Research Trust directly from ePROVIDE, using the “Online Distribution” process.
